Intake Forms

Once you have scheduled an appointment with the Babb Center, please complete the intake paperwork at least 24 hours prior to your first session by filling out the appropriate form below. If the client is a minor, and a parenting plan or other legal documentation is in place, copies of these documents must be provided at your first session.

Please note: Do not complete and submit your paperwork until you have an appointment scheduled. Call (615) 824-3772 to schedule an appointment.
